Release Forms If you are going on a trip, retreat or are taking part in an activity, please print the appropriate release form below, fill it out and bring it to the appropriate group leader. You need only fill out the General Media Release form one time General Media Release Form (required for all church activities) Youth Code of Conduct (required for all youth events) SKATEBOARD GROUP Please fill out ALL apporpriate trip release form(s) for each trip you wish to participate in.
